If you wish to register at the practice, please contact the receptionist either by phone or via the intercom in the lobby and request a registration pack and accompanying literature. Once you have completed the registration forms please return them to us so that your details can be entered onto our database. If possible, we would also like you to bring evidence of your current address (eg; current utility bill, tenancy agreement, driving licence). This helps to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us. You will be registered with the practice but will be allocated a named GP. However, you may be seen by any of the GPs at the surgery; please ask the receptionist if you wish to see a particular GP.
