ONLINE MEDICAL RECORDS

You can apply to have access to your detailed coded record via Patient Access. This is the same system you can use to book appointments online and order your repeat prescriptions.

There are a number of important features of this service which we would like to point out:

You must either, complete a form, sign it and hand it in at reception in order to apply for this access and photographic ID with evidence of current address must be shown, or you can apply online, however we would still need you to verify your ID in person if we are unable to verify electronically.

Your application must be considered by a member of clinical staff before being approved, as there are some patients for whom access is not appropriate

The processing of requests must be completed by the practice within 21 days of receiving the application.

Types of coded data available to patients will include:

  • Demographics
  • Allergies/Adverse reactions
  • Medication (dose, quantity and last issued date)
  • Immunisations
  • Test Results
  • Problems/Diagnoses
  • Coded Information from Consultations

If you wish to apply for access to this service please ask for an application pack from the surgery.

COVID vaccination status

If you have downloaded the NHS App with the intention of accessing your COVID vaccination status within your medical records please be aware that access to your online record through your GP surgery will first be necessary.  This can take up to 21 days to process so please do allow plenty of time when making travel arrangements.
